     IMPORTANT 2023-2024 ATHLETIC INFO-PLEASE READ!

     

     

       Physical-Medical History Form - -Must have a physical dated after January 1, 2023 for the 2023-2024 school year.  Physicals are DUE toTSMS by Aug. 14th, 2023. 

    Rank One - Electronic forms can be found here--complete all FIVE forms. The helmet form needs to be filled out as well. 

    These forms are all do before the start of the school year. 

    ATHLETES WILL NOT BE ALLOWED TO PARTICPATE UNTIL ALL THESE ARE COMPLETE!!!!!!!

     The girls are expected to be in either the TSMS athletic gear OR their own black shorts and plain grey t-shirt.

     Please wear your own BLACK shorts and plain GREY t-shirt if you did not purchase our options.
